About 180,000 of the mammograms were false positives, meaning that the test detected a tissue abnormality but an additional imaging or biopsy test did not support a cancer diagnosis and the women did not develop breast cancer in the year after the mammogram.

For women trying to decide with their doctor which drug to take, Mortimer said the message is this: "Both of these hormonal therapies are effective and if one causes side effects, switching to the other is reasonable".

Nonetheless, for some patients, she says, extremely aggressive treatment, even if not warranted, may be the right choice for them. Therefore, the researchers also assessed PIK3CA status in circulating tumor DNA in a subgroup of 587 patients.

Previous studies have shown that denosumab reduces bone fractures by half in women following breast cancer treatment, but researchers suspected that the drug might also improve their chances of remaining cancer-free.

This prospective, double-blind trial enrolled 3425 postmenopausal women at a median age of 64 years. She said it's possible that the post-surgery radiation is critical to preventing the cancer's recurrence and spread, though that was not studied in this research.

Although the FDA has approved adjuvant denosumab as a treatment to increase bone mass in patients on AI therapy who are at high risk for fracture, it's typically used only for those with established osteoporosis. The study was led by University of Alberta and Alberta Health Services researchers, and funded through the Canadian Institutes for Health Research and Alberta Cancer Foundation. One of the issues with this term "average" is that the majority of breast cancer occurs in women who are considered average before their diagnosis, meaning they had no known risk factors prior to being diagnosed with breast cancer.
